Client Name
- In every single form you create, whether it is an application or an overnight pass, you'll need to collect Client First Name and Client Last Name located under the Client Form Fields drop down.
- In an Intake or Lead form, the software uses these two fields to show you the name of the person that filled out your form.
- In an Other form, the software uses these two fields to view who this form belongs to and attaches it to an existing profile with the same name.
- The spelling must be exactly the same for the other form to attach to an existing profile
- It does not matter where these two form fields are, as long as they are on the form somewhere.
- Only use these form fields once per form.
- If you use them twice or more, you'll need to make sure they are Required form fields, which is explained further down in this article. If they are not required you risk the software not recognizing the name entered into the form.
- Middle Name does not matter and will not be used to sync the Form to the Profile.
Client Fields vs Form Fields
- Client Fields are information that are specific to the client and sync to the Overview tab in the Client's profile.
- Form Fields such as Text Field, Signature, etc. collect information that is viewable on the PDF version of the form once it is submitted. This information does not interact with the Client's profile.
The Difference Between Check Boxes, Radio Buttons, and Dropdowns
- Checkboxes allow the person filling out the form to select multiple options
- Radio Buttons allow the person filling out the form to only select one option from the list.
- Dropdowns allow the person filling out the form to select one option from a drop down list
How to set the options for Checkboxes, Radio Buttons, and Dropdowns
- Add the Form Field you want to use to your form
- Double click the yellow form field
- Add the options to the options box and separate them by commas
How to make a Form Field Required
- Making a form field required will force whoever is filling out the form to answer the field or they won't be able to submit the form.
- Add the Form Field you want to use to your form
- Double click the yellow form field
- Check the Required check box and click OK
How to add Page Breaks to your Forms
- Page breaks are how you can separate your forms pages so it's not a single page long document.
- Choose Insert to the left of the form builder
- Choose Page Break
How to Add your Logo or image to a Form
- You'll need to use an image hosting site for this step.
- Upload your logo or image to imgur or imgbb
- Copy the Direct Image Link or right click on the image that you uploaded and click 'Copy Image Address' in the pop up menu
- Open your One Step form and click Insert to the left of the form builder
- Choose Image and enter your Image Link into the Source box.
How to Copy and Paste a form with Form Fields
- Copy the text and form fields in the original form.
- Create a brand new form or open an existing one
- Before you paste anything into the form builder, choose Edit in the upper right of the form builder
- Uncheck Paste as Text
- Now you can paste your form into the blank form builder.
