What's an Intake Form and How is it Used?

An Intake Form is used when you want to skip the Other tab on the Client page. When an Intake Form is filled out, a Client Profile with all the information added to the form is created under the Admitted tab. You may use this if you don't need to use the built in wait list or if you're going to interview and vet potential clients outside of One Step. 

When an Intake Form is submitted, not only is a profile created, but a PDF of the form is attached to the profile it creates and a notification is sent to staff members of the facility the client is admitted to. If you choose to use an Intake Form, you won't use a Lead Form. If you used both types of forms, it would create two duplicate profiles.

  • Enter the Form Feature on the left hand side of the screen

  • Click the New Form button in the upper right corner of the feature

  • Give your form a name and choose Intake Form as the type. 
    • Make sure to add Client First Name and Client Last Name from the Client Form Fields drop down
