What is an Other Form and How is it used?

An Other form is used for forms that do not create a brand new profile in One Step. Typically these forms are used after the profile has been created by an Intake or Lead form. Other forms are often forms such as Over Night Pass Requests, Release of Information forms, etc. 

Every single Other Form will need to have Client First and Last Name as form fields. These two form fields are how the software will know which profile to attach the PDF copy of the form to. You will also be notified when an Other Form is filled out.

  • Enter the Form Feature on the left hand side of the screen

  • Click the New Form button in the upper right corner of the feature

  • Give your form a name and choose Other Form as the type. 
    • Make sure to add Client First Name and Client Last Name from the Client Form Fields drop down
