If you're planning on sending potential clients or applicants to the Other tab on the Client Page, you'll need to use a Lead Form. A lead form will create a client profile under the Other tab with all the information it collects. It will also attach a PDF to that profile of the form that was filled out and send staff members a notification that a new lead is in your system. 

If you're using a Lead Form, you will not use an Intake form type. This is because the Intake Form will create a profile under Admitted. If you used both types of forms, you would end up with two duplicate profiles.

  • Enter the Form feature on the left hand side of the screen

  • Click New in the upper right corner

  • Give your form a name and choose the Lead Form type 
    • Be sure to add Client First Name and Client Last Name from the Client Form Field Drop down!
